NEW Guidelines

1. Require documentation of BMI (Body Mass Index) for all new students and those in grades K, 2, 4, 7 and 10. The new Physical Exam form to be used this summer at your pediatrician's office is available here a PDF. Please note that the second page of this new form includes the sports clearance section for team sports participation.
2. Require all sixth grade students to receive the Tdap (Tetanus, Diphtheria and Pertussis Booster) immunization prior to returning to school in September.
3. Require a dental certification; more details on this will follow. 
4. Require a medical authorization form for all prescription and over the counter medications taken during school hours.
